48 year old male was admitted to the hospital at the middle of the night because of hematamesis after consumed alcohol in that night.And He complained of severe abdominal pain also.Immediately the erect Chest X-ray was done in this patient.Comment on his X-ray?



ANSWER - Gas Under the Diaphgram 

CAUSES
  • perforated viscus e.g. perforated duodenal ulcer, perforated diverticulum
  • iatrogenic e.g. post surgery
  • bacteria - gas producing infections
  • air originating from the lung e.g. fistula
  • gynaecological e.g. abortion, water skiing
  • peritoneal dialysis 
In this case perforated gastric or duodenal ulcer may be the cause.

    i am dr ganesh from army medical core..
    this is a case of pneumoperitonium
    x ray shows gas under diaphragm(rt)
    most probable cause perforation of peptic ulcer after binge drinking.
    treatment - laparatomy and repair
